Dental Insurance and Payment Options

At Aspire Implants and Perio we believe it should be up to the patient and their comprehensive care team to decide what treatment is necessary – not the insurance companies. We do not agree with insurance companies dictating the care of our patients. Because of this we do not participate on contracts outside of the Delta Dental of Oregon Premier groups.

Although we are not in contract with most dental insurance companies, most plans include an “Out-Of- Network” plan and will still make payments to our office. If this is the case, it is likely the patient portion will be slightly higher than that of an “In Network” office.

As a courtesy, we will process your insurance claims and do our best to estimate the amount not covered by your insurance. You are entirely responsible for knowing your own insurances coverage limitations, waiting periods, and all other exclusions – we do not act as a representative to your insurance company. All incurred charges for diagnosis, consultations, x-rays, treatment, and all other serves rendered are ultimately the responsibility of the patient regardless of insurance coverage.

  • Credit Card: Visa, MasterCard, Discover, or American Express.
  • CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
